This Miraculous Medal holy card comes with a 3/4 in. x 1/2in. pewter medal on a 20in. rhodium chain.  The prayer on the back of the card is an act of Consecration to Our Lady of the Miraculous Medal:

O Virgin Mother of God, Mary Immaculate, we dedicate and consecrate ourselves to you under the title of Our Lady of the Miraculous Medal.
